West Virginia recognizes a list of 15 serious medical conditions under the Medical Cannabis Act (SB 386, 2017). The list includes cancer; positive status for HIV or AIDS; ALS; Parkinson's disease; multiple sclerosis; damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity; epilepsy; neuropathies; Huntington's disease; Crohn's disease; post-traumatic stress disorder; intractable seizures; sickle cell anemia; severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain; and terminal illness (medical prognosis of life expectancy of approximately one year or less). OMC outlines three steps. First, complete an appointment with an OMC-registered physician via real-time video — the physician evaluates your medical situation and, if you qualify, submits the Patient Certification form directly to OMC. Second, create an account at the MyComplia patient portal, complete a New Patient Registration application, and upload the required documents (passport photo, photo ID, proof of WV residency, and the Patient Certification copy your physician provided). Third, submit the $50 annual OMC fee via online ACH or by mailing a check to OMC. Applications are reviewed within 30 days. Upon approval, your digital medical cannabis card is downloaded from MyComplia.
They are two separate transactions paid to two separate parties. The clinic charges its consultation fee at booking — that fee covers the physician's video visit and the submission of the Patient Certification form to OMC. The clinic collects no portion of the OMC fee. Separately, after certification has been submitted, you pay a $50 application fee directly to the West Virginia Department of Health through MyComplia. That $50 is the OMC's annual fee for the medical cannabis ID card. Beckley residents should plan around both costs. The $50 OMC fee is paid annually and is required for each renewal.
According to the OMC Patient Registration page, you'll need four documents: a digital photo of passport quality; a photo ID (copy of your driver's license, passport, or state ID); proof of West Virginia residency (see the WV DMV's list of acceptable residency documents); and a copy of the completed Patient Certification form your physician provides after the video visit. Once all documents are uploaded and the application submitted, a confirmation screen displays your Application ID number for tracking and payment.
OMC publishes its timeline directly: applications are reviewed within 30 days of receipt. Email updates are sent throughout the process. If approved, you'll receive an email instructing you to log into MyComplia and download your digital OMC Medical ID Card. If corrections are needed, OMC rejects the application back to you with a description of what to fix — you correct and resubmit rather than starting over. If denied, OMC notifies you by email with the reason.
The West Virginia medical cannabis card is valid for one year and must be renewed annually. Per the OMC Renewal Guide, the four renewal steps are: obtain a new annual physician's certification; log into your MyComplia account; create, complete, and submit a Patient Renewal application; and submit the $50 renewal payment. Upload the new physician certification under the Physician/Condition Information tab — most other application information carries over. Beckley patients should begin the renewal process well before their card expires to avoid any lapse in coverage. Renewals may be completed via real-time video with any OMC-registered physician.

The Two Separate Fees

The two fees go to entirely different recipients — the clinic collects none of the OMC fee.

What the WV Card Authorizes

A West Virginia medical cannabis card authorizes purchase of cannabis in specific forms only: pill, oil, topical (gels, creams, ointments), forms appropriate for vaporization or nebulization (including dry leaf for vaporization only), tincture, liquid, and dermal patch. Smoking cannabis is not authorized under West Virginia law — dry leaf or plant form may only be used in a vaporizer. The card is valid at any licensed West Virginia dispensary statewide and must be renewed annually through MyComplia.
